Requirements for Filing Trademark Applications 

1. Simply signed Power of Attorney (scan copy is sufficient);

2. Simply signed Statement of Ownership (scan copy is sufficient);

3. Trademark label;

4. Translation in English and transliteration (if the mark is written in other character);

5. List of goods/services and the class pertaining thereto.

Renewal

1. Simply signed Power of Attorney (scan copy is sufficient);

2. Simply signed Statement of Use (scan copy is sufficient);

3. Copy of registration certificate.

Recording Change of Name/Address

1. Simply signed Power of Attorney (scan copy is sufficient);

2. Document of change of name/address;

3. Copy of trademark certificate or trademark application (if yet been registered).

Recording assignment

1. Original simply signed Power of Attorney;

2. Deed of assignment notarized and legalized up to the Indonesian Embassy/Consulate in the assignee’s country;

3. Copy of trademark certificate or trademark application (if yet been registered).

Recording license agreement

1. Original simply signed Power of Attorney;

2. Copy of license agreement;

3. Copy of registration certificate.
